Congestive heart failure is a condition where the heart is unable to function. The main function of the heart is to pump blood throughout the body.

Congestive heart failure can occur as an acute or a chronic condition. The heart has four chambers. In congestive heart failure, depending on the severity of the condition it can affect either side of the heart or in some conditions the entire heart fails to function.

Two types of heart failure generally occur:

Systolic heart failure- this condition occurs when the heart fails to pump blood out from the heart.

Diastolic heart failure- this condition occurs when, the muscles of the heart are affected and the inadequate blood is filled in the heart.

Due to this the main function of pumping oxygen rich blood to the rest of the body is affected. When this occurs, fluid accumulation takes place in liver, lungs, gastrointestinal tract etc. Due to this there is lack of oxygen rich blood to various organs which affects their function.

The failure of heart can also occur due to:

– Any kind of infection which may affect the heart muscle.

Congestive heart failure can also be due to any kind of defect in the heart valves, in congenital heart disease, heart attacks and arrhythmias.

Torn Meniscus Surgery And Symptoms

The knee contains two menisci which are present in between the femur and the tibia. They are made of cartilage. Depending on their position in the knee they are divided into the medial meniscus and the lateral meniscus. The medial meniscus is located medially in the knee and the lateral meniscus laterally. The meniscus is C – shaped and wedge shaped on cross section. The peripheral surface of the menisci has a blood supply while a major central part is avascular. It allows equal body weight distribution through both the knee joints. The knee joint consists of three bones the femur, the tibia and the patella. The surfaces of these bones facing the joint are covered with a layer of cartilage. The cartilage is a protective film which prevents the bones from rubbing against each other. The meniscus lies on the cartilage in between the two bones.

Symptoms of a meniscus Tear: Following an injury the patient will complain of intense pain and swelling of the joint. Locking of the joint is also common that means the patient is unable to completely straighten the knee joint, this commonly occurs due to a loose body caused by a torn part of the cartilage. Symptoms of a meniscus tear are knee pain, swelling of the knee joint, clicking within the knee, limited range of motion of the joint, tenderness on palpation.

Causes of meniscus Tear: A tear of the meniscus is most commonly referred to as cartilage tear. The most common causes of a meniscus tear are traumatic injury to the knee joint and degenerative processes. Degeneration of the knee joint like osteoarthritis of the knee joint makes the menisci very susceptible to injury and damage. Injury to the knee when the joint is bent or twisted results in a tear of the meniscus.

Diagnosis of a Meniscus tear: A careful history and physical examination often helps to diagnose a Meniscus Tear. An x-ray may be advised to check the degree of degenerative changes in the joint. An MRI of the knee joint helps to diagnose meniscus tear and diagnose the extent of damage to the knee joint.

Treatment of Meniscus Tear: Treatment of meniscus tears varies widely from one person to another. Not all tears require surgery. Treatment depends on the type of tear, degree of damage, age of the patient, activity levels of the patient, and the response to conservative treatment. Following an acute injury the patient is advised strict bed rest, local ice application often helps to bring down the inflammation and swelling. Certain enzymes which hasten healing may be advised. For pain the patient may be put on NSAID’s. Surgery involves trimming the torn portion of the meniscus, the procedure is called meniscectomy. Meniscus repair and transplant of the meniscus are other surgical methods available for treatment of meniscus Tear.

This therapy can be given in the form of transdermal patches, gels, injections etc. testosterone can also be taken orally. However, this is not a recommended form of treatment as it gets metabolized in the liver and remains inactive there causing damage to the liver.

Whenever testosterone deficiency treatment is started, the desirable changes are not apparent immediately. It takes a good time of about 2-3 months to show effect. Symptoms gradually improve over time.

Red yeast rice is also known as the red fermented rice or red kojic rice or red koji rice or anka or ang-kak. Gestalt therapy was founded by Fritz Perls and Laura Perls and Paul Goodman in the 1940s. Chicken Pox Vaccine Side Effects and It’s Uses

Cerebellar ataxia may develop during the recovery phase or later. Women who get chicken pox during pregnancy are at a risk for congenital infection of the fetus.

Children are given two doses of the chicken pox vaccine. The first dose is administered to children at 12 months to 18 months of age and again between the ages of 4 years to 6 years of age. People over 13 years of age who have not received the vaccine and have never suffered from chicken pox must get two doses, 4 to 8 weeks apart. The vaccine does not require a booster dose later in life. It is a safe vaccine and is a preventable illness. It airborne infection and is contagious even before the rash appears, hence it is very difficult to avoid an infection following exposure. Hence if a person has not suffered from chicken pox it is advisable to take the varicella vaccination.

Candidates for Varicella vaccination: The following people can receive chicken pox vaccine if they have never suffered from chicken pox:

Health care or day care workers, children, adolescents and adults, travelers, women of childbearing age who are not pregnant – those vaccinated should avoid pregnancy for a period of 1 year following the vaccination, college students and teachers.

It may appear as a flat flesh coloured patch over the trunk. You must take heed if this sore doesn’t heal over a long period of time, bleeds spontaneously or on slightest touch, develops recurrent crusts and scabs on the surface.

Risk factors for development of Basal Cell Carcinoma: Basal Cell carcinomas are more commonly seen in men. It is more common in people having a light complexion, and is rarely seen in blacks. You may be more prone to develop this cancer if you are exposed to sun rays or ultraviolet radiation for long periods of time. People over the age of 40 are more prone to develop this cancer, as also if you have had a basal cell carcinoma in the past you have an increased chance of developing it again or in the case your family history is positive for skin cancers you may have a higher chance of developing this cancer. People exposed to high levels of arsenic in the environment seem to have a raised chance of developing this cancer.

-when the aortic dissection affects the ascending branch, it is of the type A variety.

-when the aortic dissection affects the descending branch, it is of the type B variety.

In aortic dissection a tear occurs through which the blood starts flowing. When this defect takes place some of the blood starts flowing out and some of it remains stagnant. The stagnant blood starts applying pressure on other parts of the aorta.

When aortic dissection persists for a longer duration of time, it can lead to an aneurysm or abnormal ballooning of the aorta.

-decreased sensation and mobility is seen to be associated with aortic dissection.

Diagnosis:

A chest MRI, ECG and CT scan must be advised immediately. An aortic angiography should be performed.

The patient must be hospitalized immediately before the condition gets worse. The type A aortic dissection would need surgery to fix the defect at the earliest. Type B can be treated with medications first. Anti hypertensives should be prescribed. Pain killers and beta blockers will help relieve symptoms. Valve replacement might be necessary in some conditions where aortic valve damage has occurred.

Wolff-Parkinson-White Syndrome – Symptoms And Treatment

Wolff Parkinson White syndrome is a condition of the heart in which there is an additional electrical conduction pathway or an extra electrical circuit present in the heart. As this circuit is present in addition to the normal conduction pathway of the heart, if the electrical impulses are transmitted over this extra pathway, it leads to episodes of tachycardia which means an increase in heart rate.

Causes: The normal conduction pathway of the heart includes the sino atrial node to both the atria and from there to the atrioventricular node. From the atrioventricular node the electrical impulses are transmitted to the bundle of his, which traverses through the interventricular septum. Form the bundle of his the impulses travel from the purkinje fibres into the ventricles. Thus the electrical impulses arising from the sino atrial node excite the atria and from there travel to the ventricles through the conduction system and result in the activation of the ventricles. The sino atrial node discharges impulses at a normal rate and in normal condition is the dominant pace maker of the heart muscle. The nodal discharge and conduction occurs in a regular fashion and maintains equal timing between two successive beats. In people suffering from Wolff Parkinson White syndrome, there is an additional or accessory pathway which traverses from the atria to ventricles and this result in a very rapid heart rate which means a tachycardia of the supraventricular type.

Symptoms: Some patients suffering from this condition may be totally asymptomatic. Some patients may have a few episodes of tachycardia which are widely spaced may be over a period of months to years. Other patients may complain of frequent episodes of tachycardia occurring at a rate of once to twice per week. The patient may complain of symptoms such as pain in the chest or chest discomfort or tightness, dyspnoea or shortness of breath which may or may not be related to exertion, light headedness and a feeling of dizziness, fainting and palpitations.

Diagnosis of Wolff Parkinson White Syndrome: When a patient is examined during a period of tachycardia, the heart rate may be greater than about 200 beats per minute. During the episodes the blood pressure may be normal or low. One must know that the heart rate which is normal in adults is about 60 to 100 beats per minute, around 150 beats per minute in infants, neonates and small children. In between the episodes of tachycardia the patient will be completely asymptomatic and the physical examination is completely normal. An electrocardiogram may show an abnormality called the delta wave. As the patient is asymptomatic between the episodes of tachycardia, in order to capture the episodes of tachycardia, the cardiologist may advise a 24 hour Holter monitoring machine test, which helps record the heart rate and rhythm over a period of 24 hours. Another test essential is electrophysiological studies of the heart.

Treatment of Wolff Parkinson White Syndrome: Medical treatment involves administration of antiarrhythmic medications like adenosine, amiodarone etc. In cases where the heart rate does not return to normal with the help of medication, electrical cardioversion may be done. Radiofrequency cather ablation of the accessory pathway is a preferred method of treatment nowadays. It involves ablating the extra pathway and thus restoring the normal electrical conduction of the heart. The success rates of this procedure are about 85 to 95 percent. Open heart surgery is not such a commonly used method of treatment.
